What Causes Miscarriage Without The Sign Of Vaginal Bleeding?

my wife ultrasound report shows thus,the uterus is bulky and contains a gestational sac with poor decidual reaction.no fetal node is seen.GS=2.7mm=7weeks1day.the internal os is closed.the adnexa and POD are free.The impression is missed abortion.How can this happen since there is no sign of bleeding.
