Hi,

It seems to be muscular but less likely due to pace maker. As it's subcutaneous or not that deep. Also the pain at opposite site is less likely as it doesn't radiate backwards. Local site infection should be ruled out though at lace maker site.

Meanwhile OTC analgesics like acetaminophen or Ibuprofen can be used along with muscle relaxants like Tizanidine or Orphenidine.
