General Surgeon 's  Response
Hi,

With the history of urinary tract infection all your symptoms are suggestive of the following:
There is still urinary tract infection, now there is involvement of kidneys causing classical pains on sides of abdomen and back.
Hence I would advise you the following:
send the urine for culture and sensitivity.
Routine test of blood like CBC for raised white cells, Kidney function tsts and any other investigations as would be advised.
Ultrasound of abdomen followed by CT scan of abdomen particularly for urinary system can help the proper diagnosis.
Once a proper cause is found then the treatment can be specific.
Take plenty of oral fluids.
Symptomatic treatment for pan and any other symptoms of pain, burning urination, nausea and fever.
Specific treatment say for example antibiotic as per the reports. surgery if there is any obstruction ,stone, stricture and so on.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
