What Causes No Palpable Pulse, And Un-recordable BP While Treating Head Injury?

03days back a female old lady 76 yrs came to my OPD with H/O fall and head injury with Vertigo.After I gave Vitamin Vitalise OD after 03days today again she has come to my Hospital Indoor with the complaints of Loose motion 4times,Abdominal pain & Headache.Her Vital signs are normal and motor response also good then with admission I gave Ringers Lactate 500cc Infusion 30drops per min and Injection Hyosine Butile bromide IM one ampule and Injection Metronidajole 500mg one ampule IV stat.After 02hours suddenly patient is collapsing with no BP and pulse recordable with dilatation of pupil and patient died.I want to know how patient died?Please give me some information ?

I really appreciate your concern, if this was the history of head injury then it need to be taken seriously and patient wanted to be kept under medical observation and CT head was necessary and it ought to be done, the most probable cause could be head injury, hope this information helps, take care and have a nice day.
