Hi,

There are two possibilities, one is cardiac issue for which have already been tested and underwent angiography as well, so this is out. Now remaining possibility is cervical spine disorder. This problem occurs due to compression of nerves in spine. Do you have any neck pain or trauma to neck. You should one cervical spine x-ray ap/lat view and you may need MRI spine as well.

Treatment involves physiotherapy, neck traction, ca and vit D supplements. Till the time your your evaluation is complete you can try tab neurobion forte which is a nerve vitamin.

Hope I have answered your query. Let me know if I can assist you further.
