Cyclobenzaprine is musculoskeletal relaxant which is indicated for muscle spasms. It can be given in initial dose. In elderly, cyclobenzaprine dosage needs to be reduced because they may be more prone of adverse effects than younger patients. This drug is used for only short period (not more than 3 weeks).
Gout is caused by the crystallization of urate in joints, usually monoarticular (involve single joint such as great toe, ankle, knee, wrist, finger joint). The sign and symptoms are pain and swelling, warmth,
redness in the area affected as a sign of inflammation, fever, tophi (collection of urate crystal in
soft tissue). To make a diagnosis, it's recommended to do arthrocentesis, and the joint fluid should be sent to lab for fluid analysis to find crystal microscopically.
Gout consists of 3 stages:
1. Treating acute attack
2. Providing prophylaxis to prevent acute flares
3. Lowering excess stores of urate to prevent flares
Choices of drug therapy are:
a.
Colchicine (as uricosuric agents) and NSAID (non-steroidal anti-inflammatory drugs as pain reliever), if you don't have GI problems.
b. Steroid oral (
Prednisone) and colchicine
c. Intra-articular steroid injection (
triamcinolone) plus colchicine or NSAID.
Cyclobenzaprine is not included in management of gout
arthritis. For the dosage or best choice of therapy, better consult your local doctor.
Here some tips to be concerned are:
• Maintain drinking enough water (8 glasses per day).
• Avoid food from high purine/protein source such as meat, sardines, bacon, mussels, salmon, turkey, etc.
• Avoid sodas, alcohol, food and beverages with high fructose, corn syrup sweetening since it can cause uric acid level elevation.
• It is suggested to reduce body weight in obese patient.
