A painful and swollen vein under the tongue is appearing to me either a blocked salivary duct due to formation of crystalline stones in the duct of the gland.. Other causes can be infection in the gland leading to inflamed duct of the gland.. You should consult an Oral Physician and get evaluated and a thorough clinical examination and palpating the area can help in pin pointing the cause.. A cross sectional x ray can also be done for confirmation.. In case of stone it has to be removed either manually or surgically. For inflamed duct antibiotics and anti inflammatory painkillers will be advised.. For now you can take Ibuprofen to relieve pain.. Take soft meals..
Hope this helps..
Regards.
