What Causes Pain During Pap Smear?

Hi,

My mother is having problems doing a transvaginal sonography and pap test. It hurts her a lot. Also the Urine test shows 40 to 50 pus cells. She has benign fibroids and in 2009 her transvaginal endometrial thickness was 11mm. She went through menopause in 2008 and in 2011 her fibroids have shrinked each by 7mm. Why should she have pain while doing pap smear or transvaginal tests. Is it because of UTI infection or should I worry about something else?
