I have read your question fully and understand your concerns.
From what you have written I think you are suffering with
pyelonephritis along with cystitis. Pyelonephritis is an infection of either one or both kidneys. Cystitis is infection of
urinary bladder (in other words the UTIs you had in past). It is very likely that the organisms from your cystitis moved upwards and infected your kidneys. The pyelonephritis could also be a result of a kidney stone that has become obstructed and causing infection on a background of cystitis.
Pyelonephritis causes pain in the back/flanks. There is fever with chills and rigors. Patients also have nausea and vomiting. Your cystitis is causing your lower
abdominal pain, pain and burning sensation while passing urine. UTIs also cause blood in urine. Kidney stones also cause pain.
Unfortunately, treatment of pyelonephritis is by antibiotics. I understand that you do not have access to antibiotics at present. Is it possible for you to see a doctor as soon as possible as pyelonephritis does need prompt treatment. If there is a delay in treatment it can cause complications like sepsis and acute kidney failure.
Azo may help, but may not get rid of the infection.
If you have
Co-amoxiclav (dose 650 mg three times day - provided you are not allergic to penicillin) or
Ciprofloxacin (500 mg twice a day-provided you have not allergic to it ) you could take them for first couple of days, until you get to see your doctor.Both of these are antibiotics. You need to send a urine sample ( before taking your first antibiotic dose) for culture and sensitivity to identify the correct antibiotic.
Do drink lots of water to help flush the infection. Also you can take pain relief with Ibuprofen 200 mg-400 mg three times a day (take with some food if possible, alternatively take
Ranitidine 150 mg twice a day to prevent
gastritis).
In future, as you are getting repeated UTIs, you can go on preventive treatment – by taking a small dose of an antibiotic at night.
