Hi Thank you for asking HCM. I have gone through your query. Pain on chest radiating to shoulder can be due to cardiac ischaemia. So an ECG should be taken to rule out the condition. If there is no signs in ECG then a tread mill test should be done to rule out exercise induced ischaemia. If your blood pressure is having variations like you measured Holter monitoring and ambulatory BP measurement should be taken also. A consultation with a cardiologist is well appreciated.
