Abdominal pain, hardness in abdomen and difficulty in passing stools is suggestive of
chronic constipation.
In addition, bleeding after passing stools could be due to injury to the anal mucosa while passing hard stools.
Blood can also be due to bleeding in the lower gastrointestinal tract.
Cause of constipations needs to be found out.
Constipation can occur due to lack of fibre in diet, it can occur due to some kind of obstruction in the gastrointestinal tract.
Thus I suggest you to
consult a gastroenterologist for clinical assessment and relevant investigations.
An ultrasound abdomen is suggested to look for any
organomegaly or any abnormal mass in the gastrointestinal tract.
A
colonoscopy may also be required to assess the status of lower gastrointestinal tract.
In addition clinical assessment of the anal region is also required to look for cause of anal bleeding as well as to look for cause of vaginal bleeding.
Management can be planned after complete work up.
In the meanwhile you can take stool softeners to improve the consistency of stools.
