Hello and welcome to HCM,
Pain radiating from loin or lumbar region (kidney region mentioned by you) radiating to heel of feet can be due to radiculopathy or sciatica.
Radiculopathy is involvement of spinal nerves by some disease process like compression, irritation etc. which is causing pain which starts from loin region and radiates to feet.
Sciatica is also a type of radiculopathy and it is characterized by pain radiating from loin to hip to back of thigh to side of leg and finally to feet.
Sciatic nerves is the major nerve which supplies the lower limb.
Diabetics are more prone to develop neuropathy but neuropathy usually affects the peripheral nerves.
The pain described by you is not likely to be due to neuropathy.

Thanks and take care
