Noted the history of horrible pains in stomach, trouble with bowel movements, serious swelling in hands, feet and legs, starting to get dark spots down ankles under arms and other parts of body as you have mentioned. The dark spots may be due telangiectasia or ecchymosis and the similar spots in the wall of the intestine gives the symptoms of pain, nausea, vomiting, bloating, loose stools, malena that is black stool. I would advise you the following in such a situation: Get the opinion of a hematologist for clinical evaluation, examination, investigations of the blood, stool and urine; X-ray of the abdomen in standing position, Contrast enhanced CT scan of the abdomen and so on as per the advise of the specialists like Gastroenterologist and to get an appropriate diagnosis and management.
