What Causes Pain In The Areola?

Hello, I am a 35 year old female, 67KG, 5 ft 4 I suffer from type 2 diabetes. I am experiencing pain in my left areola. It is also intermittently going very hard. It stays hard for about 1 minute and then goes back to normal. Yesterday i also noticed a lump that also appears only temporarily. It seemed to move around the circumference of my areola. yesterday was the 21st day of my menstrual cycle. I experienced this about one month ago around the same time. I thought that that may be the reason why it s hard and painful but I want to make sure it s not a serious health issue. What do you think is the problem.
