-Arthritis
-Any traumatic injury of the bone or the acromio-clavicular joint
-Also sleeping position.

I suggest you to take anti-inflammatory medication such as Ibuprofen or Naproxen daily for 1-2 weeks. Also continuing physiotherapy might help. If your pain doesn't improve, you should consult a doctor for a physical examination, to evaluate pain.
