my brother in law age 70 has had a acute change in physical symptoms, returned to bed, feeling body shakes, low left back pain, is usually up and about, now is dizzy, and doesn t want to get up, no chest pain, previous MI 12+ years ago, on blood thinner and antihypertensive therapy,

Hi,

Back pain, isolated, could suggest a wide variety of pathologies. However, when specifying the area of ​​the back that presents the pain, mention body shake (probably due to fever or chills, I assume) and dizziness led me towards a probable high urinary tract infection.
