General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

Pain during defecation and dizziness is two different things.

For pain during defecation local rectal examination needed and accordingly further work up planned

For dizziness check your blood pressure and serum glucose level to rule out blood pressure fluctuations and hypoglycemia.

Inner ear infection also can lead dizziness so otological evaluation can be done.

Lack of sleep, less eating, smoking, excess stress also related to dizziness

Take care

Hope this will help you
