General & Family Physician 's  Response
Hi,

Based on the history you have shared, it's likely muscular or neuropathic. Muscles in front of chest and base of the neck are related and supplied by the same nerves which supply the upper arm.

Many muscles have insertion and course over or around the should joint and moving an upper limb or doing work may increase the pain. X-ray cervical spine is recommended to look for any signs of stenosis or spondylosis.

Nerve compression at level of the cervical spine may cause such symptoms. If X-ray is not remarkable CT or MRI may be needed. Though less likely but a chest X-ray may be done to rule out any growth or lesion at apex of lungs.

If pain is severe and not getting relieved by OTC analgesics, then more potent analgesic like tramadol or gabapentin may be used. Muscle relaxants like tizanidine or orphenidine can be helpful.

Warm compresses should be done and not cold ones. Orthopedic review and physiotherapist visit may be needed.

Hope I have answered your query. Let me know if I can assist you further.

Take care
