Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

We will try to help you in best way possible. First of all it's sad to know what your son had been going through lately. Based on the history you have shared, it be muscular and relation to heart seems very less likely as you suspected. An orthopedic surgeon review and X-ray shoulder joint may be needed though muscular lesions doesn't show up on X-rays and frozen shoulder or rotator cuff syndrome are most likely possibilities and usually resolve with in few months with rest, guided physiotherapy. Using OTC analgesics like Acetaminophen can also help.

Hope I have answered your query. Let me know if I can assist you further.
