What Causes Painful Arms In A Disabled Person?

My husband is disabled and uses a walker to get around. He woke up today with severe pain in left arm from elbow to fingers. Cannot use left hand and seems to be crippled up. We thought it might be gout but meds not helping. He is 75 has afib,previous open heart surgery,gout and is hunchback from several back surgeries. Noheadache,no facial problems.balance a little off. What should I do??
