What Causes Painful Defecation And Blood In Stool?

I have an ongoing and rather embarrassing issue with bowel movements that I have been dealing with for many years and slowly gets worse. I am not sure what can be done about it. I have never spoken with anyone about it, but I am not sure how much longer I can take it. My movements are EXTREMELY painful and there is blood (bright red) and over time out of fear of this pain I have developed a tendency to hold it in until I can't take it anymore. This of course makes it worse but nothing I do can make it less painful. What can I do?

Detailed answer..
As there are small veins at the anal area and when there is passing of hard stool , these ruptures and cause bleeding during defecation .First of all I would like to tell you that it is very common and can be treated .All you have to do is avoid chicken , mutton and increase fibre diet..like bananas , raw vegetables , cucumber, carrots and avoid rice.As increase fibre in diet helps treating constipation and softens stool.For treatment apply xylocaine gel, 2 spoons of spaghol (rusk)in one glass of water befor sleeping daily and syp.lactulose before sleeping.Do visit and discuss with general surgeon for anal fissure.
