What Causes Painful Knot On Thumb And Swelling Of Wrist?

Hi on my left wrist just on the thumb side I have a very hard knot that is painful with swelling around the whole wrist I have a dimple on the top of the wrist and the knot and swelling goes under the wrist about to where you would check your pulse this has been going on for about 10 months with no improvement
