What Causes Penis Swelling With Foreskin Discharges?

Hello. My 5 year old son is having a slight discomfort on his penis since 2 days ago. There are some white discharge under the foreskin and he says it's a little painful when he touches it but not at all painful when he wees. It is red and looks a little bit swolen. Is it a kind of infection due to hygiene problem? What can I do?
