What Causes Persistent Abdominal Pain And Nausea?

My 14 yr old daughter was in the ER last night. Symptoms were abdominal pain in the center to the right quadrant and nausea. She had labs done and and a CT. It took morphine take take the pain level down to a 3. Everything came back negative. Labs were good. However, the MD said he didn t know the source of her pain. Her pain went back up to an 8. He ordered a shot of toradol and sent us home. She felt better till approximately 1815 when the nausea started again. She had eaten at 1715 with no problems. At 2030 her pain was back at an 8. What do I need to ask about? An ultrasound? What are the possibilities? An ovarian cyst?

General Surgeon 's  Response
Hi,

Normal CT scan rules out many a things like ovarian cyst with twist, appendicitis, abdominal lymph nodes, aneurysm. CT scan cannot rule out gastroenteritis as she was vomiting. Other reasons can be as follows:
- Worms in intestine
- IBS that is irritable bowel syndrome
- Colitis and Typhlitis that is inflamed cecum
- Appendiculoliths that is stone / fecolith in appendix that cause spasmodic pain
- Urinary tract infection unless has a stone in right ureter

Hence, I would advise your daughter the following:
- Consult a general surgeon or go back to ER for proper clinical evaluation and investigations
- Stool routine, microscopy
- Urine
- Tests of blood

These can help to get a proper diagnosis and thus allow a specific treatment and not just symptomatic one.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
