Hi,

History of bloated look of the lower abdomen but sometimes flat as you have described can be due to the following factors:
Intestinal problems like sub-acute intestinal obstruction.
Intususception, that is part of the intestine goes into intestine.
SIBO that is sub-acute intestinal infection.
Kinking of intestine.
Herniation through a large hernia.
Hence I would advise you the following:
When the lower abdomen is distended then go for clinical examination.
X-ray of abdomen in standing position.
Tests of stool for 3 days.
CT scan of abdomen.
Colonoscopy if needed.
All these will give a proper diagnosis and thus allow a specific management as per the final diagnosis.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
