What Causes Persistent Breakthrough Bleeding While On Jolessa?

Hi, my name is Kaylee. I ve been on the birth control Jolessa for almost 6 months now. I had a bit of spotting and breakthrough bleeding with my first 3 month pack... I am on my second 3 month pack (oral pills) and I have about 3 days before I start the pills that allow my period to happen.First, I wanted to know that when I start the pills that allow my period to happen if I ll still be protected from pregnancy while on the period pills?Second, I have been having almost CONSTANT breakthrough bleeding ALL THE WAY through this second, three month pack. With let up for maybe, 3 days every 2 weeks. It s not new red blood like regular period blood... it s old looking blood, a darker almost dry looking blood... I guess almost the same color as a dried scab. But it is the same consistency as regular period blood. I have tried douching, and it flushes out huge clots which I thought would stop the bleeding, but it did not.I NEED to know if I should switch to a different birth control to stop this. I have been considering the Depo shot. I do not have any medical need to be on birth control, but I am sexually active and do not want children. Thanks in advance for your input.

General Surgeon 's  Response
Hello,

As per my clinical experience, the Jolessa is a combination contraceptive containing Ethinyl estradiol and Levonorgestrel. This molecule has many of the side effects and the major one is breakthrough bleeding or inter menstrual bleeding. As there is side effect, the same would be happening with other brand of combined OC pills. The same will be issue with Vaginal ring and Intradermal patch which contain combined OC material as above.

In my experience, I do suggest an intrauterine device with Levonorgestrel, which will help two ways:
- There will be no issue of breakthrough bleeding or inter-menstrual spotting
- It will decrease the amount of menstrual flow as well through its effects over uterine endometrium

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
