Noted the short history related to your husband diagnosed as having Malaria and given Coartem 80/480, taken 4 tablet so far but the fever has not settled, has oral ulcers, unable to take food and sleep is not possible. I would advise you the following in such a situation: Get him admitted to the hospital for proper recording of the temperature hart to know the character/pattern of fever. Intravenous fluids to cope with dehydration and electrolyte imbalance as he can not take orally. Intravenous antibiotics if indicated. Send the blood for culture and other other tests like CBC, liver, kidney functions. To rule out diabetes. Send urine for routine, microscopy, culture and sensitivity before starting antibiotics. Other tests like X-ray chest and Ultrasonography to rule out other causes of fever. Obviously the treatment for Malaria will continue on. All these things with daily review by Doctors will help to find out the cause and get treated accordingly, have proper fluid and electrolyte balance. Multivitamins, Folic acid and additional supplements to get over the oral ulcers, local hygiene of oral cavity to heal the ulcers. Hope I have answered your query.
