Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
I am prescribed percocet 10/325mg, take 1 tab Q4H ATC. I have tremendous pain, hence the med and dosing. I would like to safely titrate down but do not want to endure any additional pain associated with the titration. My physician recommends a longer acting med such as MS04...I have declined this and all other opiates at this time. I am on disability and would like to return to work in the near future and take care of patients vs being one...
Pain is quite complex and can be from many causes (injury, disease, nerve malfunction, chemical damage, etc). Chronic use of opioid pain medication can also lead to tolerance so that you require higher doses and can cause "hyperalgesia" when the body feels more pain than usual. Without knowing the exact cause of your pain, it would be difficult to give you specific information.
Some general advice may help though. There are chronic pain medications that are not opioids and do not have issues with addiction, tolerance, and hyperslgesia. Duloxetine, gabapentin, pregabalin, nortriptyline, and others. Pain Management specialists can often do procedures to help the pain like nerve blocks. There is a new treatment that is benefiting patients called low-dose naltrexone. Psychotherapy can also teach people how to cope and deal with a chronic issue.
Take care. Hope I have answered your question. Let me know if I can assist you further.
Regards,
Dr. Timothy Wong, General & Family Physician
