General Surgeon 's  Response
Hello,

* As per my clinical experience, the persistent severe pain lower back while operated for L 4 disc removal may be in relation with

- nerve compression at the underlying foramina level of the vertebra
- chronic ligament spasm
- hypersensitivity issue for pain threshold
- irritation via unhealed bony prominences

* It is the MRI of the lower spinal region , which helps great to diagnose and manage accordingly.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Bhagyesh V. Patel
