What Causes Persistent Swelling In Legs Despite Taking Water Pills?

I'm a 56 yr old female, I went through the change about 10 yrs ago. I've been on pain medication for arthritis in knees and hands for about 20 yrs. I'm also on water pills for swelling in my lower legs. This had helped along with me propping my feet up a couple of times a day. Over the last month or more the swelling won't seem to go down. A dr here told me to double up on my water pills to twice a day and it isn't helping. What could be causing this ?
