What Causes Prolonged Periods With Clot Discharge?

i have continuous period for last 3 months. heavy clots are going. i went to see the doctor after 2 months and did scan and pap smear... all are normal. Doctor prescribed me Dicynone for two days and told me to stop it once the bleeding is over. Bleeding reduced after taking 3 days for 3 tablets a day. Then spots started and again heavy bleeding started with clots. I am 40, married with one child. Weight 98kg and height 155cm. could you please advise what is my issue. I am fedup with this. Ultrasound scan said the following: Uterus is anteverted normal in size and shape and texture. It measures 6.81*3.95*4.28cm. no focal lesion in the myometrium. Normal midline endometrium seen. Endometrial thickness is 1.09cm. Both ovaries shows multiple small follices arranged along the periphery stroma appear thikened,, suggestive pco. Right ovary measures:2.4*1.72 cm and left ovary measures 2.87*1.65cm. no free fluid is seen in POD. PAP report said : Satisfactory LBC smear. No inflamatory changes are seen. No cytological atypia detected. No specifiica pathogenic micor-organizms identified.
