What Causes Prominent Pulsation On The Neck Of A Child?

I noticed a pretty distinguishable pulsation in my childs right side of his neck back in March. he was born a preemie and a twin to a sister at 35 weeks. I never noticed it before until last march and he is 5 now. I recorded it, but kinda thought it was nothing. I am going to be a FNP and I recently studied JVD and I am kinda worried now. He is sick today, c/o really bad belly pain and tried to eat breakfast and puked twice. Wont eat anything really and now he has a fever. I checked him out for appendicitis and his psoas and obturator tests are negative for referred pain and he has no rebound tenderness. However he is sleeping and I happened to notice the vein again, bulging. He is pretty tachy, but I attribute that to his fever. I listened w my stethoscope and it sounded like he has a bruit. I got scared. HELP!!!? Wondering if I should take him to the hospital?

Hi.

Noted the detailed history about your child and as you are becoming FNP your findings are most welcome.
My thoughts.
The Pulsation on one side of neck with bruit sound on auscultation can be due to some problem with artery in the neck.
This can be best diagnosed by the Color Doppler studies and evaluation by a Cardiologist.
If anything is found to be positive full further evaluation will help whether he needs an active treatment now or later and go for the advised. Always take a second opinion of another Specialist in the said field.
As for the pain in abdomen, vomiting and fever with negative Psoas test, no rebound the cause can be anything related to worms, colitis, enteritis and so on.
Needs a clinical evaluation by Pediatric Surgeon, Ultrasonography, tests of blood, urine and stool.
Further management as per the cause found.
