Diagnosed with high blood pressure several years ago and been on a beta-blocker med to control it. That worked fine until a few months ago when my BP began to drop into the 90/50 range. I was advised to take my meds on an as needed basis. I can go for days with my BP fluctuating between 180/100 range then drop to the 90/ 50 range. When I experience this drop it is accompanied by dizziness and lightheaded feeling, with a noticeable pressure behind my ear drums. What can cause this erratic fluctuation? How can I be both hypertensive and Hypotensive?

Your blood pressure medications need readjustment. Most probably longer acting drug usage will reduce the incidence of fluctuations in blood pressure.
Other causes of fluctuations are diabetes related nervous dysfunction(if you have diabetes), intermittent infection or dehydration. Over dosage of blood pressure medications (short acting medicines mainly)
So, my advice to you is - you should do 24 hrs blood pressure monitoring at your home (ambulatory device is available) which will guide your doctor exactly regarding the fluctuations in blood pressure.
Your doctor can readjust your blood pressure medications dosage or change the medicines also.. There is no condition like hypotension and hypotension- combined. So don't worry and go for the test I have advised. Thank you. Have a nice day.
