What Causes Fluctuating Blood Pressure Levels?

I live in Utah and my Doctor is in Calif. I have been having trouble with my blood pressure it ranges from 150 to 225 the lower part is around 50 to 80. I am 84 yrs old, female. When I go to Calif. to see my Doctor my blood pressure is normal 133/70. Can you tell me what causes this?
