What Causes Rashes On The Upper Torso And Neck?

My brother is a known drug user. He has Hep-C. He just got out of jail last week. This past weekend he developed a rash on upper torso, neck and inside mouth. His right hand is obviously swollen and red. Parents took him to walk-in clinic. His blood pressure was low but his pulse rate was high. Nurse Pract said to go to ER. Well he refused due to lack of insurance. I saw him yesterday and he seemed better but still had swollen right hand and I believe he still had rash... Parents are at a loss, he is a 55 year old male. Any ideas?

-take antihistamines daily
-us calmine lotion for local relief
-if you don't get better take prescribed corticosteroids from your doctor
