What Causes Rectal Bleeding And Pain During Bowel Movement?

My sixteen-year-old daughter has been having intermittent anal or rectal bleeding and pain during bowel movements. She says she can feel tissue pushed out and that she can push it back in and that does temporarily relieve the pain. What should I do?
