What Causes Recurring Pain In Kidney And Chest With Shortness Of Breath?

My boyfriend is having some really bad symptoms and it is scaring me. He wakes up daily and says he feels terrible, he is always short of breath, he has pain in his kidney area and his lower back, he gets these chest pains that dont last long but they are pretty frequent, he gets dizzy, he has numbness in his arms and hands at times, he is ALWAYS tired and has ZERO energy, he doesnt have much of an appetite anymore at all, and he went for a physical for a job and they found blood in his urine. He was laid off and lost his insurance so he wont go to the doctor cause he cant afford it so he is being stubborn. I of course am looking online and it doesnt look good but does anyone have any ideas of what it could be?
