What Causes Recurring Skin Tear On Penis?

my husband has skin tears on his penis.his dr thought it was a yeast infection,he used the prescribed meds. it healed then we had sex it came back...we have been devoted to each other over 30+ yrs,he had contact with a hiv pt when he was a police officer over 18 yrs ago ,,,,antibiotci cream heals then when we have sex oral etc it comes back..he says it hurts also
