What Causes Red And Painful Toe Which Shoots Up Till My Leg?

Hi I am 49 5.5 and about 13 stone. For nearly a year now I have had a "hole" in my toe underneath, the chiropodist removed a peice of grit but this didnt alleviate it. It is currently red, painful and sometimes pains shoot up my leg. Wonder if there is any advice please?
