What Causes Red Patches And Tightness In Foreskin Leading To Pain?

My pennis foreskin was long and i am able to see the inner gland only at ejaculation time. That too only the tip of the gland. One day I found a white powder like formation and irritation. I tried to clean it. From next day a red colour patch formed on my foreskin with enormous pain. I am not at all able to retract my foreskin and irritation while urination is found. I visited a dermatologist and he prescribed a fluconzole tab and ketoconzole ointment for a period of 4 days. Irritation stopped for 2 days. Then I found red patch again at different location every 2 days. Now nearly last 10 days I am suffering and able to retract the foreskin at all and unable to see the gland.
