What Causes Reduced EGFR Value While On Hydralazine?
My question is regarding a 77yr old white female with elevated hypertension. Her egfr 2 weeks ago was 48. She was started on hydralazine 50mg tid and spironolactone 25mg daily. Yesterday s repeat labs showed a K of 5.1 and egfr now is 31. Should I speak to my colleague about this rapid change or adjust the hydralazine myself?
Hello and welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.
As per your description, it appears that the patient is having hypertensive nephropathy. You need to ascertain first by clinical evaluation as to whether the drugs have contributed to drop in eGFR or the kidneys have not responded to the medications and the condition has worsened. Hence, kindly consult Urologist promptly for clinical evaluation and appropriate management.
Hope the information provided helps. All the best.
Regards,
Dr. Ashakiran. S
