What Causes Reduced Amniotic Fluid During 30th Week Of Pregnancy?
Dear Dr., My wife is 30 weeks pregnant and is suffering from low amnotic fluid level which decreased to 4-5 ( as per the Radiologist X s report). She was admitted for Hermin IV drips 200ml (one daily for 4 days). Also being given following: 1.Arginine (sachet twice daily) 2.Becadexamin (1 daily) 3.Cap Evion 400mg( 1 daily) 4.Tab Corcium(twice daily) 5.Tab ASA(1daily) 6.Inj Prolotun 500mg (once a week) 7. Cap Alamin M Forte (1 daily) 8. Two shots of Betnesol inj. 9.Inj Amikacin. 10. Tab Thyronorm. After 4 drips AFI level increased to 7-8 (Doppler Test done by Dr.Y). But after a week in next Color Doppler AFI level dropped to 5 (Doppler Test done by Dr X) . She was again admitted for Hermin IV 200ml drips (1 each for 4 days). Again after 4 days Dr. Z reads AFI level as 9.5. This time Radiologist (Dr Z) shows us that she is seeing some good fluid pockets inside and thats the reason it is 9.5. Meanwhile she continued her daily medications and weekly Injections. After 3 days of Doppler test when the AFI level was 9.5, we took doppler test again ( with Dr. X) and now she said AFI has dropped to 2-3. But when i informed her that 3 days ago it was 9.5 as per other doctor s report. We went for second opinion with a 4th Radiologist (Dr. Q) and this test was taken with in 90 mins of the earlier doppler test (done by Dr X). Dr. Q reads the AFI level as 7.2 as he shows us pockets around baby. Per him it is not as low as 2 - 3 and we can monitor is after 5 days. Another difference in two reports were the positioning of baby wherein earlier it was Cephalic and later it was breech and that too got changed within 90 mins. Does baby positioning affects in detecting the AFI levels or there could be some other reasons as there is gross difference between two doppler tests taken with in 90 mins of interval. - Is there any harm in increasing Arginine intake from twice daily to thrice daily - Does Alivher help instead of Tab ASA and how much should be the dosage. As per my surgical experience, positioning of the baby would changes the fluid pockets positioning and difficult visualization to the radiologist when 2 D sonogram is used in some of the cases. But with the advent of 5 D sonography, excellent evaluation is possible in all the fetal positions with equal efficacy. The discrepency in reporting of AFI values can be part of absorption, swallowing by fetus or less urination during last 90 minutes as compared to previous scan or observer differences. However, a clinical examination and discussion with the concerned radiologist is best to obtain in this critical case.
