The inability to urinate is probably due to prostatic enlargement or some kind of obstruction.
Prostatic examination is required to determine whether there is any prostatic enlargement or not.
In case,
prostate is enlarged, a
biopsy is required to determine the cause of enlargement.
Most common cause of prostatic enlargement is benign
prostatic hyperplasia (
BPH).
Prostatic cancer needs to be ruled out since prostatic cancer can also cause enlargement of prostate.
In addition, serum PSA levels also need to be determined.
Normal range for PSA of less than 4 ng/mL, however, in case of cancers, PSA level increases.
Further management can be planned once result of investigations is out.
BPH is primarily managed surgically, however, for symptomatic relief i.e. allowing passage of urine drugs may be helpful.
