What Causes Right Hand Trembling And Pain In Shoulder And Lower Back?

My right hand has been severely trembling for the past two hours when i open it. I have a slight pain in the left side of my head and neck. My right shoulder and lower back is also flaring with pain. I feel a slight pressure on chest when i breathe and feel a little fuzzy in the head. Should I go to the emergency room?
