What Causes Right Shoulder Pain Radiating Towards Chest Despite Taking Advil?

Should I be concerned of something more serious, if I have pain in my right shoulder blade that goes through to my upper chest and is now in my neck? I took an Advil a couple of hours ago but it has not helped. It has been this way since about 3:00 a.m. No better but other than traveling to my neck, no worse. Not a sharp unbearable pain, just steady.
