Hello, my blood pressure is acting strange. This morning it was 86/59, this evening it was 147/129.I was very dizzy and felt exhausted this morning.I am not on any type of blood pressure meds. I have been having some pain and discomfort in my upper chest above my right breast, middle of my chest to my back, down my shoulder and arm and sometimes up the right side of my neck into my lower jaw area. I have being somewhat dizzy/lightheaded off and on for a few weeks, however today is the first time I ve checked my BP in awhile. I am a 52 year old female. Your response would be appreciated.

Thanks for the question posted by you,

In general, there are many causes create chest pain potentially. Gastrointestinal tract disorders account for about 40% of chest pain reasons such as peptic ulcer, esophagitis, gastritis, etc. About 30% of underlying causes of chest pain originate from cardiovascular system such as ischemic heart diseases or coronary artery problems, cardiac arrhythmias, cardiac valves abnormalities, etc. About 25% of causes for chest pain arise from musculoskeletal problems such as rib cage pain, osteoporosis, muscle pains, etc.
According to your chief complaint including chest pain, dizziness, being exhausted and blood pressure changes, etc. you might be suffering from a cardiovascular problem.
To address exactly the underlying mechanisms explaining your current health problems, I recommend the following orders:
1. Test your cardiac situation and reserve using ECG (electrocardiography) and cardiac stress tests based your physical exams done by cardiologist.
2. Echocardiography, after you receive the test results mentioned above.
3. You may demand to take a chest x-ray (CXR) to rule out other causes which mimic chest pains and also to assess your heart size.
4. Holter monitoring (24hours) in case of some specific cardiac arrhythmias.
5. You are advised to write down your Blood Pressure regularly during day for some weeks to follow up hemodynamic instabilities.
6. In very rare occasions you may need to assess your chest pain using CT scan based on physical examination performed by your doctor and tests results.
7. You need to test your urine and blood using routine tests such as urinalysis, CBC, Hb, Hct, FBS, TG, HDL, LDL, etc.
8. Because of your symptoms and age, you are strongly suggested to take a bilateral breast sonography, and also, you may demand a mammography digital to rule out breast tumor, however, the probability in your case is very low.
Please notice that all the tests mentioned above are not essential to pursue at the same
time, the sequence would be ordered after visiting your doctor.

Hope you find my recommendations helpful,

Regards
