Considering your father's background of being a chronic diabetic and persistently elevated
creatinine levels, he may be suffering from the
chronic kidney disease.
This explains why he is having difficulty of breathing,
pleural effusion, electrolyte abnormalities like low sodium and high potassium levels, as well as swelling on the extremities known as edema.
Although aging is associated with a normal decrease in kindey function, being diabetic (especially if uncontrolled) is a known risk factor for chonic kidney disease.
Creatinine level elevations may be due to the progression of his kidney problem on top of uncontrolled sugar levels, considering a creatinine level above 5 is already very high and may warrant renal replacement therapy to prevent complications.
I humbly suggest a consult with a kidney specialist (
nephrologist) for proper diagnosis and management, as well as to know the best treatment options for your father's case.
I also suggest tight blood sugar control to prevent risks of elevated sugar levels on other organs.
