What Causes Rise In Creatinine Level Of An Elderly Person?

My father who is a chronic diabetic was admitted to hospital for breathing problem. His creatinine level is above 5 only for the past few years. He was diagnosed to be suffering from pleural effusion and put on lasix immediately. He was high on potassium level (5.9) and low on sodium level(119). He was given K Bind and nodosis. He was given oxygen support for two days. Breathing became normal and swelling in foot, fore arm and face vanished. After checking the potassium and sodium levels, he was discharged. He was advised to continue lasix for a few more days. Now his creatinine has gone up (7.8). But he appears to be getting better and is weak now. Lasix dosage has been reduced from today. Test will be repeated after one week. Is it abnormal rise? He is 85 years old.

General & Family Physician 's  Response
Hello,

Considering your father's background of being a chronic diabetic and persistently elevated creatinine levels, he may be suffering from the chronic kidney disease.

This explains why he is having difficulty of breathing, pleural effusion, electrolyte abnormalities like low sodium and high potassium levels, as well as swelling on the extremities known as edema.

Although aging is associated with a normal decrease in kindey function, being diabetic (especially if uncontrolled) is a known risk factor for chonic kidney disease.

Creatinine level elevations may be due to the progression of his kidney problem on top of uncontrolled sugar levels, considering a creatinine level above 5 is already very high and may warrant renal replacement therapy to prevent complications.

I humbly suggest a consult with a kidney specialist (nephrologist) for proper diagnosis and management, as well as to know the best treatment options for your father's case.

I also suggest tight blood sugar control to prevent risks of elevated sugar levels on other organs.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
