What Causes Scabbing On An Old Surgical Incision?

I have a e year old incision that has never healed properly..is hasa scab, which will flake off, then make a new one... it is on my legjust abovethe ankle. Afterthe surgery the leg became infected... had to haa second surgery as it got to the hardware in my ankle. They ran rests and found I had VRE and I hI hd to be isolate for six weeks during which titime I was give antibiotics to cure the infection. I hve had general discomfort ever since...swelling (worse in summer_ and some pain, but it is tolerable. able. When removing the scale, I have found stitches , along with the scaling tissue...two different occasions. TLast night it was painful even to hav my leg resting on the mattress...so I checkedthe leg this morning, and there is a hole in the incision. I am 84, legally blind , live alone, but afriend stopped by and I had her look at it. She confirmed t the hole and said she saw what appeared to be infection. I am wondering if I should go to the ER to have itchecked out. I thought to ask for advice here before I call my doctor.

General & Family Physician 's  Response
Hello,

Since you have a history of recurrent infection at the incision site, you do need to visit ER, at the earliest. You have not written whether you are diabetic; if diabetic you should get treated for diabetes first, diabetes can interfere or trigger wound healing. If you are prescribed with medications, take the doses as suggested by your treating doctor. Keep your blood sugar under control.

Get physically examined, it looks like you need intravenous support of antibiotics & fluids. Get all blood & Imaging tests done. Your treatment will depend upon the severity of infection.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Nupur K, General & Family Physician
