What Causes Secondary Amenorrhoea?

Nov 2010 was the last period on my own. Took Provera March 2011 and had a period. Took it again but didn't have one till May 12, 2011. Have taken Provera 3 times since and no period. Once, I spotted when I was suppose to have one. Lately, I have intense cramps, every day. I always feel like a period is coming, but it doesn't. I am not pregnant and the doctor did an ultrasound and biopsy and cleared me of tumors etc. Prolactin, tsh, cbc, cmp all normal. I am 36 and NEVER had issues with periods, never even late before. I am a normal weight and do not overly exercise or diet. Also, the only thing the ultrasound revealed was a cyst that recently burst. My tummy was hard and looked 4 months pregnant, then after the burst it went away. But that was 2 months ago, I should of had a period by now.
