Hello and welcome to Ask A Doctor service. I read your query and here is my answer.
Can you describe the pain? Is there any pain in urination or frequent urination? Does he run fever? Have you done any tests like blood tests or ultrasound abdomen etc? As you said he is having pain which radiates to the left shoulder means that something is irritating his diaphragm or phrenic nerve. He may need ultrasound abdomen to rule out any intraabdominal hernia and renal problem.
Hope i answered your question. Feel free to ask if you need further assistance. Thanks.
